Visit Confirmation For Shoulder Patients

Please read the following information carefully and ensure that all steps are completed before your appointment

1. Before Your Visit

  • If you have a cough, cold, flu, respiratory illness, or COVID-19 please do not come to the office.  Let our office know in advance of your appointment and we will reschedule your appointment in a timely fashion.
  • Masks are NOT MANDATORY, but our office staff wear masks in accordance with Lakeridge Health’s current policy which we also follow.  We strongly encourage patients to do so within our office space.

2. What To Bring With You

  • Ontario Health Card
  • Information on your referring physician (Name, Address, FAX)
  • Information on your preferred pharmacy (Name and FAX)
  • Give any recently completed Xray, Ultrasound, CT or MRI reports and a link to your imaging online (Such as PocketHealth) or CD-ROM (your doctor may have sent some reports in advance but you may have had more investigations since then and we would like to be up-to-date)
  • Glasses which you may require to fill out any forms
  • Face Mask which completely covers your nose and chin (NOT MANDATORY BUT RECOMMENDED)

If you require a caregiver or translator, they are welcome to attend your appointment with you.  As space in the waiting room and examination rooms is limited, we ask that you only bring one other person if required.

3. What To Wear

If you are seeing us for your Shoulder, Elbow, Wrist, or Hand

Wear a tank top or T-shirt which will allow access to the back, side, and front of your shoulder if required. 

If possible, please come to your appointment wearing your garment.  If you would like to change on-site, washrooms are accessible on our floor in advance of your visit.

If you are seeing us for your Hip, Knee, Ankle, or Foot

Wear a pair of shorts, track pants, tearaway pants, or loose pants which can be pulled up to mid-thigh level (for knee) or mid-shin level (for foot and ankle).

If possible, please come to your appointment wearing your garment.  If you would like to change on-site, washrooms are accessible on our floor in advance of your visit.
